Before enrolling their child, parents must complete and sign a contract and registration form. These forms gather essential personal details about the child, including their name, date of birth, address, emergency contact information, parental responsibilities, dietary requirements, fee and session preferences, as well as contact details for parents, doctors, and health visitors, along with information on allergies, parental consent, and vaccination records.
